Eligibility Criteria

Individuals must fulfill the following qualifying requirements in order to use our housing stabilization services:

  • Be 18 years or older
  • Be on Medical Assistance (MA)
  • Have a disability or disabling condition
  • Possess a history of unstable housing, which includes being homeless, leaving an institution, or being in danger of becoming such.
  • Require assistance in at least one of the following areas due to a disability
  • Communication
  • Mobility
  • Decision making
  • Managing challenging behaviors
