Eligibility Criteria
Individuals must fulfill the following qualifying requirements in order to use our housing stabilization services:

Be 18 years or older

Be on Medical Assistance (MA)

Have a disability or disabling condition

Possess a history of unstable housing, which includes being homeless, leaving an institution, or being in danger of becoming such.
