What Are Intervention Support Services?

Intervention support services are designed to help individuals improve daily living skills, social interaction, and independence while reducing behavioral challenges. At CareTeam Home, we provide both in-home support (in the client’s residence) and out-of-home support (in community-based or residential settings).

Our services are tailored to:

Types of Intervention Support Services

In-Home Intervention Support

Delivered in the comfort of the individual’s home, this service ensures personalized care and guidance in familiar surroundings.

  • Assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and meals.
  • Skill-building support: communication, problem-solving, and social interaction.
  • Behavior management strategies to reduce disruptive or unsafe behaviors.
  • Structured routines and therapeutic activities.
  • Family training and caregiver support to reinforce positive outcomes.

Out-of-Home Intervention Support

Provided in community-based or residential settings, this option allows individuals to develop skills in new environments.

  • Recreational activities, educational support, and independent living training.
  • Respite opportunities for families while ensuring the individual is engaged and supported.
