CareTeam Home is a 245D Licensed Provider committed to helping individuals avoid institutional care by delivering customized, person-centered services in the comfort of their homes and communities.
We operate under the Minnesota 245D Waiver Program, part of the state’s Home and Community-Based Services (HCBS) initiative. This program is designed to support seniors and individuals with disabilities in living independently while receiving the care and support they need.

At CareTeam Home, clients have the flexibility to choose from our standby team of qualified professionals, (RN, LPN,CNA and PCA) each dedicated to promoting a more independent and fulfilling life. Our staff work closely with clients and county case managers to set up and manage 245D Waiver services, ensuring a seamless and supportive experience.

24-Hour Awake Care
Unlike live in care, where caregivers may rest during the night, 24-Hour Awake Care ensures continuous supervision. This service is designed for individuals who cannot be left alone due to medical conditions, mobility issues, dementia, Alzheimer’s, or high fall risk.
Caregivers work in shifts to stay awake and responsive at all times monitoring, assisting, and supporting your loved one whenever needed.
Homemaker Services
Homemaker services focus on non-medical assistance with household chores and daily activities that individuals may find difficult due to age, illness, disability, or recovery from surgery.
These services do not replace medical care, but they ensure that the home remains a safe and supportive place to live.
Elderly Companion Services
Elderly companion care is a non-medical support service that focuses on enhancing quality of life through friendship, conversation, and assistance with everyday tasks. While not providing hands on medical care, our companions help seniors stay socially active, emotionally supported, and engaged in daily life.
Nighttime Monitoring Services
Nighttime monitoring provides continuous supervision and support during overnight hours. A trained caregiver stays awake and alert throughout the night to ensure safety, respond to needs, and provide comfort whenever required.
Unlike traditional “sleep over” care, our caregivers remain awake and vigilant to quickly assist with any situation whether it’s a trip to the bathroom, medication reminders, or responding to an emergency.
Respite Care Services
Respite care is temporary, short-term relief for caregivers, allowing them to rest, attend personal matters, or simply recharge. During this time, CareTeam Home ensures that the care recipient continues to receive the highest level of support, supervision, and assistance.
Individual Community Living Supports (ICLS)
ICLS is a comprehensive, person-centered service designed to support individuals with daily living activities, decision-making, and community participation. Unlike traditional in-home care, ICLS focuses on helping clients gain skills, build confidence, and enhance independence in both home and community settings.

In-Home and Out-of-Home Intervention Support Services
Intervention support services are designed to help individuals improve daily living skills, social interaction, and independence while reducing behavioral challenges. At CareTeam Home, we provide both in-home support (in the client’s residence) and out-of-home support (in community-based or residential settings).
Individualized Home Supports (IHS) With Training
Individualized Home Supports with Training provides hands-on instruction and guidance in a person’s home or community setting. Unlike traditional care that focuses mainly on supervision or assistance, IHS with Training emphasizes teaching skills that foster independence and self-reliance.
Employment Support Services
Employment Support Services are specialized programs that provide career development, job placement, and ongoing workplace support for individuals who need assistance entering or remaining in the workforce.
Individualized Home Supports (IHS)
Individualized Home Supports (IHS) are services delivered in a person’s home or community setting to provide assistance, guidance, and skill-building in daily living.
