245D Waivered Services in Minnesota

Care Team Home makes it easy to begin 245D waivered services for individuals and families in Minnesota. We support clients receiving CADI, Elderly Waiver, and other Home and Community‑Based Services (HCBS) with compassionate, person‑centered care.

✅ Step 1: Obtain an HCBS Waiver

To begin, contact your County Human Services Office and request a MnCHOICES Assessment.
This assessment determines eligibility for HCBS waivers, including CADI, DD,BI and the Elderly Waiver.

✅ Step 2: Work with a Case Manager or Social Worker

After your assessment:

A county or managed care case manager will review your results
You will select the HCBS waiver that best meets your needs
You will choose services available under your approved waiver
Your case manager will help coordinate services and provider referrals.

✅ Step 3: Start Services with Care Team Home

Once your waiver services are approved, contact Care Team Home directly or ask your case manager to refer you.
